New Patient Registration

Patients wishing to register with Sperrin Family Practice should bring their NHS medical card to the Practice and those of any other persons registering with you. If you do not have a medical card, you can call at the surgery and collect registration form.

Persons applying for National Health Service registration for the first time are obliged to present their passport or work permit which we will photocopy and send with HS22X form.
Alternatively if you have been registered with the NHS before, please use the HS200 form available from reception.

The Practice also requires a recent utility bill and photographic ID. It is also useful if all children under the age of 7 could have a copy of their childhood immunisations/vaccinations to give to the Practice

Non-Registered Patients

The practice will provide immediately necessary treatment for patients who are not registered. When the practice provides any clinical services to a patient who is not on it’s list of  patients, it shall, with the patients consent  provide a clinical report which will be sent to the patient’s own doctor.

 

Change of Personal Details

It is very important to inform us of any change of name, address or telephone number to allow us to keep our records   updated.  Evidence of a name changed is required e.g. birth     certificate or marriage certificate.
