Patients wishing to register with Sperrin Family Practice should make contact with the Practice. If patients are within the Practice boundary they will be advised regarding the relevant registration forms to be completed and the supporting documentation required.

Non-Registered Patients

The practice can provide immediately necessary treatment for patients who are not registered. When the practice provides any clinical services to a patient who is not on it’s list of  patients, it shall, with the patients consent  provide a clinical report which will be sent to the patient’s own doctor.

Change of Personal Details

It is very important to inform us of any change of name, address, email or telephone number to allow us to keep our records  updated.  Evidence of a name changed is required e.g. birth certificate or marriage certificate.

Medical Records

Patients have the right to request copies of their medical records and will be asked to put their request in writing to the Practice and that the relevant consent forms will need to be completed prior to disclosure of records

Registering a birth

To register a birth in Northern Ireland, you must fill in the form below. 

(Note that a birth registration cannot be easily changed, so make sure to fill in the registration form carefully)

Once you have given this form into your local registration office, the registrar will give you an Infant Registration Form (HS123). You can use this yellow form to register your baby as a patient with your GP practice. Please bring the HS123 infant form into the GP Practice as soon as possible  so that your baby can be registered with the GP well in advance of the 6- 8 week check up and vaccines.

Registering a death

You can register the death if you are a relative, a witness to the death, a hospital administrator or the person making the arrangements with the funeral directors.

Temporary Registration/Immediately Necessary Treatment

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice. We do not have to accept you as a temporary patient although we do have an obligation to offer emergency treatment. You cannot register as a temporary patient if you are already registered at a practice in the town or area where you are already registered.

Foreign Language Interpreting Support

The Business Services Organisation (BSO) Interpreting Service is commissioned by the Department of Health/SPPG to provide foreign language interpreting support to Health and Social Care (HSC) providers in Northern Ireland (NI).  The main aim of the Service is to improve access to HSC Services for Patients/Clients who do not speak English proficiently. HSC Interpreting is demand-led and requests vary according to the needs of Patients/Service Users. Providing an Interpreter:

  • ensures clear, accurate and impartial communication
  • minimises risk of misdiagnosis, misunderstanding and non-consent; reduces times/costs associated with repeat appointments, prolonged appointments, unnecessary admission to hospital;
  • complies with legislative requirements: Race Relations (NI) Order 1997; Section 75 of the Northern Ireland Act 1998
